Notes:

Payment by cheques, one for at least 25% of total fees are due on registration day, the remaining balance due by post-dated cheque on or before December 31, 2015.

$33.00 Mandatory Annual Skate Canada Registration and Insurance Fee (one per skater)

$40.00 WOS Promotional Contest ticket book (one per family)
